The W-2 Box 12 Reporting Rule
Box 12 can contain up to four entries, each with a letter code and a dollar amount. Some codes report amounts already excluded from Box 1 (taxable wages). Others are informational only, or included in Box 1 but requiring separate identification.
How Box 12 Codes Work
- Codes D, E, G, AA, BB, EE are pre-tax retirement deferrals — already excluded from Box 1 taxable wages.
- Code W (HSA employer contributions) is excluded from wages and not subject to income or payroll tax.
- Code DD (employer health coverage cost) is informational only — not taxable, not deductible, no action needed.
- Code C (group-term life over $50,000) is included in Box 1 — it is taxable income already counted in your wages.
- Box 12 holds a maximum of four separate code/amount entries. Employers may issue additional W-2s if more are needed.

Scenario: Reading Your Box 12
James receives his W-2 and sees three Box 12 entries: D $21,000 / W $1,500 / DD $9,200.
James does not need to do anything with DD. His 401(k) and HSA contributions have already reduced his taxable wages — both are reflected correctly in Box 1.
