W-2 Box 12 Codes Explained

Box 12 on your W-2 reports compensation that doesn't fit standard wage boxes — retirement deferrals, health coverage costs, employer HSA contributions, and more. Each code tells the IRS something specific about how that amount is taxed.

The W-2 Box 12 Reporting Rule

Box 12 can contain up to four entries, each with a letter code and a dollar amount. Some codes report amounts already excluded from Box 1 (taxable wages). Others are informational only, or included in Box 1 but requiring separate identification.

Scenario — Reading Your W-2 Box 12

James receives his W-2 and sees three Box 12 entries: D $21,000 / W $1,500 / DD $9,200.

Code D = $21,000 → 401(k) pre-tax deferral. Already excluded from Box 1 wages. Code W = $1,500 → Employer HSA contribution. Not taxable, not in Box 1. Code DD = $9,200 → Total health insurance cost. Informational only — ignore for taxes.

James does not need to do anything with DD. His 401(k) and HSA contributions have already reduced his taxable wages — both are reflected correctly in Box 1.
